Release of Information and Acknowledgement
I hereby give permission to the BMBA to use the above information in confidentiality and as required for administration purposes or in the case of emergency. I, the undersigned, hereby give permission of the above mentioned child to participate in the Bradford Minor Baseball Association. I understand that BMBA, the Executive, umpires and coaches can not be held responsible for injuries incurred during the normal course of a game or practice or while traveling to a game or practice.
NOTES
All fees must be paid in full before a child is allowed to play softball.
Returned cheques are subject to a $50.00 service charge. Non-payment voids registration.
Payments received after March 31 are subject to a $30 service charge.
Refund requests must be made in writing. All refunds are at the discretion of the Executive.
Approved refunds provided after March 31 are subject to a $50 service charge.
Outside of scheduled registration sessions, completed registration forms and cheques must be mailed to
BMBA Box 1344 - Station Main, Bradford, Ontario L3Z-2B7. Make cheques payable to "BMBA".
